How To Establish An Account

Open an account with a brokerage firm, bank, or other custodian. You may begin with any combination of cash or marketable securities. You may also ask us to manage an existing account.

Our Kopp Investment Advisors sign-up forms are available for viewing and printing as interactive PDF (Portable Document Format) files. Please save a copy of the PDF file to your local hard drive using the "Save" icon on the Acrobat toolbar. If the file name is displayed as "*.pdf" you will also have to rename the file. To fill out the form electronically, close your internet browser window, launch Acrobat Reader and open the file you just saved. After filling in all requested information, please save, print out, sign and return a hard copy of the forms to us at the address listed on the first page of the form packet.

Confidential Client Questionnaire, Investment Management Agreement and Limited Power of Attorney: Please fill out the Confidential Client Questionnaire completely and sign as indicated. This questionnaire helps you analyze whether our management style is appropriate for you. Read the Investment Management Agreement carefully and sign where indicated. This agreement describes the legal relationship between us. Please also read and sign the Limited Power of Attorney. This form authorizes us to buy and sell securities on your behalf and notifies a brokerage firm that we have the power to make investment decisions on your behalf with respect to the account.

Read and sign the Certification of Investment Powers only if the account is in the name of a trust, corporation or other entity.
Addendum to Investment Management Agreement for Investment Limited Partnerships: This form must be signed by the general partners of a limited partnership account or by an authorized representative of a limited liability company account.

You may also be asked to review and complete additional forms relating to special types of accounts or clients.

Upon receiving documents completed by you, we will:

Review the information and may contact you or your broker or financial professional, for clarification or additional information.
Begin to invest or re-invest your account as you have directed.
Communicate with you from time to time.

Let us know if you wish to add cash or securities to the account.

Our Investment Management Agreement may be terminated at any time by either of us without penalty. Upon termination, you will receive a pro-rated refund of any prepaid management fee.
